Web-based health questionnaire (£75)
For those who want a quick, convenient expert comment on their health, lifestyle and risk areas, TDC have developed a questionnaire based health assessment, to:
- assess your likely overall state of health
- identify any indicators to current and potential health issues
- indicate the likely causal factors behind specific health issues
- provide a quantified health score in key risk areas.
Based on the responses, TDC’s doctors will produce a report covering the above factors, recommend steps towards better health management and, if necessary, recommend areas for further investigation.
The questionnaire can be downloaded here or on request, can be posted to you.
It should be completed and returned (by email or post) to TDC. Postal returns should be accompanied by a cheque for £75.

Your health report will be produced and despatched within three working days.
